Supervisor, Health Access Enhanced Care Management, RN (Riverside) - Remote

Under the general supervision of the Health Access ECM Manager, the Supervisor, Health Access Enhanced Care Management, RN (RN Supervisor) will be responsible to partner in program development and implementation of the care management operations, and wholistic concerns of the members, including physical behavioral and social health. The RN Supervisor is responsible for overseeing daily operations of the care management teams which will also include clinical (according to their licensure), administrative, and operational reputed company. This position engages the care Team Members to work collaboratively to build and maintain high levels of engagement, as well as an excellent Member experience. This position will reputed company RNs, licensed and non-licensed Behavioral Health Team Members, Coordinators, and Community Health Workers (CHWs), and other Care Team Members reputed company required. Education & Requirements

• Three (3) years or more experience in acute care or Behavioral Health Care field
• Two (2) years or more in a reputed company or supervisory reputed company
• Associates Degree in Nursing from an accredited institution required
• Bachelor’s degree in Nursing from an accredited institution preferred
• Certified Case Manager or other reputed company certifications preferred
• Possession of an active, unrestricted, and unencumbered Registered Nurse (RN) license issued by the California BRN required

Key Qualifications

• Must have a valid California Driver’s license and valid automobile insurance. Must qualify and maintain driving record to drive company vehicles based on IEHP insurance standards of no more than three (3) points
• Deep understanding and knowledge of services for Members (Adult and Youth) experiencing following:
• Homelessness
• High Utilizers with frequent hospital admissions, short -team Skilled Nursing Facility stays and/or Emergency Room visits
• Serious Mental Illness (SMI)
• Substance use Disorder (SUD)
• Adults, youth, and children transitioning from incarceration
• Birth Equity
• Other Population of Focus as deemed as necessary by the State
• The knowledge to support mental health, substance use conditions, and acute /chronic medical health needs
• Awareness of the impact of unmitigated bias and judgement
• Skilled in evidenced based communication such as Motivational Interviewing, or similar reputed company-based communication strategies
• Understanding of and sensitivity to multi-cultural community
• Knowledge of whole person health and the effects of social deprivation
• Excellent relationship skills
• Highly skilled in interpersonal communication, including resolving conflict with co-workers/Team Members
• The ability to reputed company at an intermediate level of reputed company and Word is preferrable
• Skillful in informally and formally sharing expertise
• Ability to work well in leading teams and provide supportive assistance as needed
• Must have the skills and reputed company to tolerate change as needed and reputed company required to promote improvements
• Supervisor will work remotely and, in the field, as needed to meet with Team Members/Providers as needed
• Minimal physical activity; may include standing and repetitive motion
• Position is eligible for telecommuting/remote work location upon completing the necessary steps and receiving HR approval
