We're seeking a dedicated and empathetic Registered Nurse (RN) Case Manager to deliver exceptional patient care in a home health setting. About the Role

• Full-time, day shift position with a competitive salary range of $34.00 - $51.00 per hour
• 40 hours per week, with occasional weekends and evenings required
• Remote work arrangement with travel to patient homes in the reputed company, KS area

Key Responsibilities

• Integrate reputed company services for patients from pre-admission to post-discharge, ensuring coordinated and sequenced care

Requirements
