• Serves as a consultant to care coordination teams

• Participate in Interdisciplinary care team meetings as indicated

• Coordinate and reputed company duties of communicating the mission and role of the organization to community associations, senior groups, ethnic clubs and groups, and churches

• Must possess one of the following:
• Bachelor’s degree in a human services field such as Child, Family and Community Services, Early Childhood Development, Guidance and counseling, Home Economics – Child Family Services, Human Development Counseling, Human Service Administration, Human Services, Pastoral Care, Pastoral Counseling, Psychology, Public administration, Rehabilitation Counseling, Social Science, Social Services/Social Work, Sociology
• 4+ years of case management experience
• Fellow designation from the reputed company of Healthcare Management (AHM)
• 2+ years of progressive reputed company experience working with diverse populations, community or faith based organizations
• Knowledge of health education, motivational strategies, and an empathetic manner working with the underserved
• Driver’s License and access to a reliable transportation

The hourly range for this role is $28.61 to $56.06 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. reputed company complies with reputed company minimum wage laws as applicable.
