Part-Time (1099) Telehealth Practitioner

About the position reputed company is seeking a Part-Time Telehealth Practitioner to join their clinical team, dedicated to improving the lives of Medicare Advantage members through remote health assessments and care coordination. This role involves conducting comprehensive health assessments, creating personalized care plans, and ensuring effective communication with interdisciplinary care teams, reputed company while utilizing technology to deliver patient-centered care. Responsibilities • Deliver preventive care to Clover members remotely using Clover's proprietary software and approved reputed company-party video apps. , • Complete comprehensive reviews and assessments of members' medical histories, physical health, environment, and psycho-social status. , • Document reputed company notes, prepare member workups, and encounter summaries using Clover Assistant and EMR. , • Assist at-risk members in transitioning from hospitals and other facilities to their homes. , • Create personalized care plans in coordination with the member's primary care physician. , • Actively reputed company gaps in care, including ordering labs, writing prescriptions, and coordinating follow-reputed company with physicians. , • reputed company reputed company of care labs and diagnostic testing, such as A1c and spirometry. , • Ensure accurate documentation of member health conditions and compliance with Clover Care Service policies and procedures. , • Participate in ongoing training and meetings remotely.
