Job Description: The Social Worker (LMSW) will provide critical support to patients by assessing their social, emotional, and environmental needs to ensure appropriate care reputed company the hospital and swing bed program. The social worker will conduct comprehensive assessments to establish personalized Plans of Care, collaborating with reputed company providers to ensure patients needs are met. This role involves assisting patients reputed company the hospital setting and facilitating referrals to external resources for further support. Key Responsibilities:

• Assess patients for appropriateness to the Critical Assess setting, ensuring reputed company criteria are met for hospital and/or swing bed program admission.
• Conduct comprehensive assessments of patients' needs, strengths, goals, life history, and preferences.
