Primary Responsibilities: • Exception to meet KPI 50 calls a day • Assess patient's health status and recommend care based on clinical judgment and protocols • Identify potential care and/or provider gaps • Coach patients on treatment alternatives • Coordinate services and referrals to health programs and community services • Assesses and triage immediate health concerns • Initiate the plan of treatment/care pathway based upon the analysis of information obtained and reputed company scope of practice and certification • Accurately communicate (written/verbal) patient status information to prescribing physician and case manager in timely manner and provide necessary clinical information to reputed company pharmacists and other reputed company as applicable • Provide care and services to assigned caseload in an efficient, organized manner.
