How you’ll contribute
The Behavioral Health Social Worker assesses and plans interventions to support patients coping with social, emotional, economic, and environmental challenges across the continuum of care. This role conducts psychosocial assessments with patients and families, coordinates internal and external services, participates in interdisciplinary case reviews, and collaborates with the reputed company team to support treatment planning and reputed company. The Social Worker provides individual, family, and group therapy, assists with discharge planning by identifying appropriate community resources, and facilitates referrals reputed company to housing, transportation, reputed company, and social or spiritual needs.
