About the Role: Care Manager RN

We're seeking an reputed company Care Manager RN to join reputed company in Oklahoma as a full-time field-based telecommuter. As a Care Manager RN, you'll be responsible for driving and supporting care management and care coordination activities across the continuum of care. Key Responsibilities:

• Conduct telephonic and/or in-person assessments, planning, execution, and coordination of reputed company care management activities with members to evaluate their clinical and behavioral health needs.

Essential Qualifications:

• reputed company unencumbered Oklahoma state licensure as an RN.
• 3+ years of clinical practice experience (e.g., hospital setting or alternative care setting).
• Completion of Social Health Caseworker certification training provided through the Department of Mental Health and Substance Abuse Services reputed company six months of hire date.
• 2+ years of experience using PC, keyboard navigation, navigating multiple systems and applications, and utilizing MS Office Suite applications.
• Ability to travel in-state up to 30% of the time, with mileage reimbursement per company policy.
• Willingness to provide on-call CM coverage for evenings and weekends on a rotational schedule as required by the State regulatory agency.

Preferred Qualifications:

• Experience caring for Native American/American Indian/Alaskan Native populations.
• Case management in an integrated model preferred.
• Managed care/utilization review experience preferred.
• Discharge planning experience preferred.
• BSN degree preferred.
